THE ADMISSIONS PROCESS
Step 1: Complete the Enrollment Interest Form.
Step 2: Submit Complete Application (including supporting documents): Complete all aspects of the application listed on the “Application checklist” (below). Supporting documents may require you to make arrangements with your current school’s Secretary of Admissions.
Step 3: Complete Moorpark Unified Enrollment Process: This process will be detailed during the New Student Orientation:

APPLICATION CHECKLIST
- Submit Current Transcripts
- Students Applying for 9th Grade:
- All Middle School Transcripts
- All High School Transcripts (if applicable)
- All College Transcripts (if applicable)
- Students Applying for 10th, 11th, or 12th Grade:
- All High School Transcripts
- All College Transcripts (if applicable)
- Students Applying for 9th Grade:
- Immunization Records
- Submit Additional School Information
- List of currently enrolled courses (at all institutions)
- Attendance Report from current school
- Discipline/Behavior Report from current school
- Standardized Test Scores (CAASPP, Private School, Out of State, if applicable)
- Gate, 504 Plan, or Individualized Education Plan (IEP) documents (if applicable)
